A cold prologue plants the 1964 spree
The film opens in 1964 inside the corridors of Fairwater Sanatorium, prowling from a victim's point of view. A pleading woman whispers "Please don't hurt me." Off-screen voices declaim "The sins of the flesh will ruin her" and "The wicked will be punished." The original killings — orderly Johnny Bartlett's twenty-seven-minute spree through the hospital, no patient or staff spared, not even those at prayer in the chapel — are staged without yet being named.b1 A chapel detail is planted that the film's climax will cash out ninety minutes later.
The action moves to present-day Fairwater. At the Cornerstone newspaper, editor Magda Rees-Jones spikes reporter Steve's draft about a returning Grim Reaper — over thirty unexplained heart-attack deaths in four years, the "shadow of Death" descending again on a town synonymous with the "Bradley-Bartlett murder spree."b2
Frank runs the con
Frank Bannister — widowed ex-architect, current "psychic investigator" — drives away from a stranger's funeral with three ghosts in his trunk and crashes through Ray Lynskey's fence.b3 Ray's wife Lucy, the town doctor, makes a house call on the Bradleys — Patricia, a timid woman in her mid-forties, and her domineering mother — and the mother turns her away with the dismissal that her daughter is "not to be trusted."b4 That night Ray and Lucy watch a TV documentary on the 1964 spree: Bartlett's "twenty-seven blood-soaked minutes," twelve people murdered, the chapel killings, and Patricia Bradley — fifteen at the time, "madly in love with the psychopath," sentenced to life and released on conditional parole "five years ago."b5
Frank's ghost crew — Cyrus (a 1970s street ghost), Stuart (a nerd from the 1950s), and the Judge (an Old-West gunslinger) — fakes a haunting at the Lynskey house.b6 Frank arrives to "exorcise" his own confederates with practiced patter, and mid-spiel sees a luminous "37" pulsing on Ray's forehead, visible only to him.b7 The con is the equilibrium; the number is the warning. A second con at the Waterhouse house follows, where newspaper editor Magda confronts Frank on the sidewalk: "Preying on the bereaved is about as low as you can go. You're a parasite, Mr. Bannister."b9
The case finds Frank
Outside the Lynskey house Frank stumbles into Ray, now a confused ghost — Ray has just died of a "heart attack," "a viselike grip just squeezing my heart."b10 At the cemetery the ghostly Master Sergeant Hiles rages at Frank for "using spooks to put the frighteners on people," while Sheriff Walt warns him the FBI is interested in the unexplained heart-attack pattern.b11
At the restaurant where Lucy was supposed to celebrate her wedding anniversary, Frank stops conning. He tells her he was in a car accident five years ago that "altered his perception," relays Ray's hovering confession honestly — Ray blew her $16,000 savings on a bad investment — and lets the marriage's failure stand.b12 In the men's room he sees a robed figure with a featureless face reach through a man's chest and crush the heart; a heavenly light briefly opens and Frank whispers "Mom."b13
Dammers arrives
FBI Special Agent Milton Dammers — twitching, lank-haired, twenty years undercover with cults — interrogates Lucy at the station and then walks Frank through the 1990 Debra Bannister case in forensic detail.b14 b15 Frank, a "successful architect" before Debra's death, had promised her a garden in the new home and instead poured a concrete slab for a basketball court. The car argument, Debra demanding to be let out, the curve, the body fifteen yards from the wreck. Dammers ends on the detail that ties Debra to the present-day case: "Debra's corpse had the number 13 carved into its forehead."b15
Frank tracks the Reaper's column-of-life beam to a museum, where Magda is working late and now has the next number on her forehead. The Judge materializes and drives the Reaper off with his ghostly six-shooters — then is scythed mid-distraction while leering at a museum mummy. The Reaper carries Magda out of the museum; Frank pursues in his car and finds her body off Holloway Road. Her ghost initially blames him — "You killed me, you bastard!"b17
Dammers interrogates Frank with the Nina Kulagina psychic-killer theory and accuses him of all twenty-eight unexplained deaths since 1990, "starting with your beautiful wife, Debra. She was the first."b19 Lucy meanwhile finds Frank's missing seven-blade utility knife — the one from the Debra crime scene — hidden in Old Lady Bradley's closet, and confronts Frank in his cell: "You're a goddamn hypocrite."b22 b23 The Reaper attacks Frank through the cell wall; Cyrus and Stuart throw themselves at it and are torn apart.b24
The freezer
Frank, his crew gone, tells Lucy what comes next: "I got to have an out-of-body experience. And I got to have it right now."b25 In a clinic side-room she lowers him into a domestic freezer with Lidocaine and a twenty-minute window. The transactional operator becomes a participant; every post-midpoint beat flows from this voluntary crossing.
Dammers walks in, drags Lucy out, and drives her toward the cemetery while Frank's body lies past the safe-revival window. Intercut, Patricia stabs her mother in her bedroom with a kitchen knife and returns downstairs with the lie that "Mother wants to talk to the police... she'll be down in a minute."b26 Ghost-Frank breaks free of his body, meets Hiles in a spectral cemetery — "Sound off like you've got a pair!" — and corners the Reaper in a vault.b27 b28 The cape comes off. The face underneath is Johnny Bartlett: "I thought guys like you fried in hell." "I got out, Frank. I've been carrying on the good work. Got me a score of forty."b28
The Bradley house, the urn, the chapel
Lucy revives Frank with adrenaline. He names Bartlett and tells her to get Patricia out of the house — still misreading her as the next victim.b29 Inside the Bradley house, Patricia oscillates in front of Lucy between the captive voice she has used all film and a second voice tallying body counts with Bartlett's ghost: "That'll give us forty-one. That's eight clear of Gacy."b30 Lucy finds Old Lady Bradley dead in her bed. Frank meets Lucy as she runs, retrieves the urn — "These are Johnny Bartlett's ashes" — and names the destination: the chapel of the old sanatorium.b31
Inside the abandoned hospital the 1964 killings bleed through the corridors: a spectral doctor, a young Bartlett shooting him in his orderly's whites, a teenage Patricia in the hallway. Bartlett turns to her: "What's the score, Patty?" Her answer — "Eleven" — settles what the public record never proved. Patricia was an active participant in the original spree, not just the implicated lover.b32
Patricia hunts Lucy through the ruins. Dammers gets the drop on Frank in a chapel-adjacent room, seizes the urn, and drops it — "Oops" — releasing Bartlett.b34 Bartlett kills Dammers; Patricia strangles Frank in the chapel while Bartlett cheers her on: "Baby, you are an artist. You're 42."b35 b36
Climax and after
Frank's freed spirit catches Patricia's soul as it leaves her body — "I got your girlfriend, Johnny! Come and get her!" — and hauls her toward Heaven. Bartlett follows because he cannot stand to lose her.b37a A demonic worm-mouth opens beneath the chapel floor and pulls Bartlett and Patricia, screaming, into Hell as Heaven opens behind Frank.b37b
In Heaven the Judge, Cyrus, and Stuart greet Frank — "The old express bus to hell. No lines, no waiting." Debra appears and tells him to "start living, dude."b38 On his porch a restored Frank sits with Lucy as Sheriff Walt mentions Patricia's Ouija boards — "a direct line to her dead boyfriend as soon as she was released five years ago."b39 The unfinished house has been demolished off-screen. Lucy looks at the porch and notices Dammers's sullen ghost, and quotes Frank's old line back at him: "Sometimes, Frank, you see when you go through a traumatic experience it kind of alters your perception." Frank answers: "No."b40
